Scouts
Forms required by Troop 2 (see details below):
- BSA Registration Form – Required once upon joining. These can be obtained from the council office or at regular weekly meetings.
- Troop 2 Information Packet – Updated yearly in January.
- BSA Four Part Medical Form – Update yearly from date of physical exam.
- Colorado State Medical Form – Updated yearly from date of physical exam.
These forms must be updated as indicated and current or we cannot allow Scouts to attend outing and activities outside our normal weekly meetings.
BSA Registration Form:
The BSA registration is a multipart carbon form that is turned to the local Council office. This information is used to add the Scout to their database and attached that record to our troop. Advancements are then tracked for each Scout and the records are used to “recharter” the Troop each January. This form is required only once for any Scout that joins Troop 2.
Troop 2 Information Packet:
The Troop information packet is a collection of contact and other background information. It includes permission to attend Meetings and Permission for Emergency Medical Treatment while attending Meetings. This packet is an Adobe PDF file that can be filled and printed. You can also print this form and fill it out by hand.
BSA Four Part Medical Form:
All parts of this form are not required for every outing. The information below is to help you understand which parts are required and when. Typically this information is good from 1 year from the date next to the signature on the form and must be up to date through the end of the outing for which that form is used. Please note, in the recent past (2010-11) BSA changed its medical forms, renaming part B to "part C" and part C to "part B". Please pay close attention to the forms required for each specific outing or activity. Also, please only use the new forms to prevent confusion.
- The new BSA form parts A&B require no doctor’s signature and are needed for outings and activities that involve overnight stays and are less than 72 hours, and any activity away from the normal troop meetings. BSA Health Form Part A&B.
- The new BSA form part C is the one requiring a physical and a doctor’s signature. Parts A, B & C are required for longer term camps that are 72 hours or greater. For summer camp, the state of Colorado also requires a separate Colorado state form that also requires a physical and a doctor’s signature. BSA Health Form Parts A,B&C.
Colorado Medical Form.
- The new BSA form part D is also required for all high adventure bases such as Philmont, Northern Tier or Seabase. Scout must be 13 or 14 to qualify to go on these outings. You will be specifically informed if an outing requires this form and it will be collected specifically for that outing so you will not need this form for Troop 2 activities until instructed. BSA Form Part D only - Philmont, Northern Tier, Seabase, and Summit Bechtel Reserve.
Adults
Need same forms as Scouts. Adults attending camping overnights or driving Scouts must be registered Scout Leaders and have current Medical forms. Committee Members who aren’t attending campouts do not need Medical Forms
